Differences between proximal bone remodeling in femoral revisions for aseptic loosening and periprosthetic fractures using the Wagner SL stem

Abstract Background Monoblock taper fluted stems have been reliably used to treat proximal femoral periprosthetic fractures (PFF) and aseptic loosening (AL). Although remodeling has observed around the Wagner Self-Locking (SL) stem, exact characteristics of this process are yet be established. Our aim was compare that takes place after revisions for PFF AL. Methods Consecutive patients between January 2015 December 2017 undergoing revision using SL stem or AL without an extended trochanteric osteotomy (ETO) bone grafting were selected from our database. Radiological follow-up performed plain antero-posterior hip radiographs taken postoperatively at 3, 6, 12 months 24 months. The Global Score (GRxS) utilized by four blinded observers. Intra interobserver variability calculated. Secondary outcome measures included Oxford Hip Visual Analog Scale pain. Results We identified 20 database, 10 cases. severity Paprosky 2 in cases, 3A cases 3B 6. classified as Vancouver B2 7 B3 3 Patients regained 89% (GRxS: 17.7/20) their stock 6 months, whilst with AL, required almost years achieve similar reconstitution bony architecture 86% 17.1/20). Inter-observer reproducibility numerical GRxS values showed a “good” correlation 0.68, intra-observer agreement “very good” 0.89. Except immediate revision, we found significant difference results two groups each timepoint pair-wise comparisons. Functional groups. not able show functional results. Conclusions Proximal reconstitutes much quicker PFF, than where is ETO. accuracy measurements on good reproducibility, making it suitable everyday use arthroplasty practice.
